CMS Medicaid & Medicare Data Collection

CMS proposes to extend or revise the following data collection forms:

1. Home and Community Based Waiver Requests and Supporting Regulations in 42 CFR 440.180 and 441.300-310 (extension)
2. Consolidated Renal Operations in a Web Enabled Network (CROWNWeb) Third-party Submission Authorization Form (new)
3. Medicare Enrollment Application (revised)

To be assured consideration, comments and recommendations must be submitted by September 2, 2008.
