Almost 500 patients enrolled in a program called Guided Care averaged 24% fewer hospital days, 37% fewer nursing home days, and 15% fewer emergency department visits, Bruce Leff, MD, of the Johns Hopkins University School of Medicine, and colleagues wrote in the August issue of the American Journal of Managed Care.
